Agreement to Terms
Welcome to drmayraalfonso.com (the “Website”), owned and operated by Mayra Alfonso Physiatry, PLLC (“we,” “us,” or “our”). These Terms of Service (“Terms”) govern your access to and use of the Website and any services offered by Mayra Alfonso Physiatry, PLLC.
By accessing or using the Website, submitting a Website form, scheduling a complimentary fit screen, or purchasing or participating in a service, you agree to these Terms and our Privacy Policy. Additional terms may apply to specific services through a separate signed agreement. If there is a conflict between these Terms and a signed service agreement, the signed service agreement will control.
No medical advice on the Website. Content on the Website is for general informational purposes only and is not medical advice. For emergencies, call 911 immediately or go to the nearest emergency department.
Description of Services
Mayra Alfonso Physiatry, PLLC offers physician-led, non-emergency services focused on home safety, mobility, everyday function, and aging-in-place planning for older adults and their families. In-person services are available in Boca Raton and nearby communities. Telemedicine services may be available when appropriate and legally permitted.
The Home Safety & Viability Support Programs are private-pay, non-covered services that may include in-home observation, practical planning, family education, individualized written recommendations, and time-limited follow-up, depending on the program selected. These programs are intended to complement—not replace—the services provided by the individual’s primary care physician, specialists, therapists, home health providers, or other existing professionals.
No Physician–Patient Relationship Until Accepted
Use of this Website, submission of a Website form, or scheduling of a complimentary fit screen does not create a physician-patient relationship. Any professional relationship is established only after the applicable intake process has been completed, required agreements and consents have been signed, and Dr. Alfonso has accepted the individual for the specific services to be provided.
Communications; No Emergencies; Do Not Send Sensitive Information
The Website, Website forms, email, text messaging, and other general contact methods are not intended for urgent or emergency communications. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Do not submit urgent or highly sensitive medical information through Website forms, text messages, or unsecured email. These communications may not be encrypted and may not be appropriate for confidential clinical information.
User Responsibilities
By using the Website or participating in any service, you agree to:
- Provide accurate, complete, and current information during scheduling, intake, and service delivery
- Use the Website and services only for lawful purposes
- Refrain from transmitting unlawful, threatening, defamatory, harassing, infringing, or malicious content
- Refrain from attempting to interfere with the security, availability, or proper functioning of the Website
- Follow any additional responsibilities stated in the applicable signed service agreement
Payments, Medicare Billing, Cancellations, and Refunds
A. Private-Pay Concierge Programs
The Home Safety & Viability Support Programs are non-covered, private-pay services and are not billed to Medicare or other insurance plans. Program fees are disclosed before enrollment and are governed by the applicable signed Concierge Agreement.
B. Separately Provided Physician Services
If a medically necessary physician service is separately provided, it may be billed to Medicare or other applicable insurance in accordance with coverage requirements. Any separately billed physician service is distinct from the concierge program. Applicable deductibles, coinsurance, or copayments remain the client’s responsibility.
C. Advance Beneficiary Notices
When required by Medicare rules, an Advance Beneficiary Notice of Noncoverage or other applicable notice may be provided before a service is furnished. An ABN is not used automatically for every private-pay concierge service.
D. Payment
Private-pay fees are due according to the payment schedule stated in the signed Concierge Agreement. Payment may be processed through a third-party payment platform. The payment processor’s terms and privacy practices apply to payment information handled through that platform.
E. Cancellations, Rescheduling, and Refunds
Cancellation, rescheduling, refund, and no-show terms are stated in the applicable signed Concierge Agreement. Clients should review that agreement carefully before enrollment.
F. Completed Services and Written Materials
Fees are not refundable for services already provided or written materials already prepared or delivered, except as otherwise stated in the signed Concierge Agreement or required by law.
G. Controlling Agreement
Additional program, payment, cancellation, and refund terms may appear in the signed Concierge Agreement. If these Terms conflict with the signed Concierge Agreement, the signed Concierge Agreement will control.
Intellectual Property
All content on this Website, including text, graphics, logos, photographs, program names, downloadable materials, and other original content, is owned by or licensed to Mayra Alfonso Physiatry, PLLC and is protected by applicable intellectual property laws. You may view and use Website content for personal, noncommercial purposes only. You may not copy, reproduce, distribute, modify, publish, display, sell, or otherwise use Website content without prior written permission, except as permitted by law.
Third-Party Links and Tools
The Website may include links to third-party websites, scheduling platforms, payment processors, secure forms, or other external tools.
