Focused Guidance for a Specific Concern
A focused, one-time program for older adults and families who have a specific concern related to home safety, mobility, or aging in place and need timely, practical direction. This program is designed to provide clarity around one primary issue without the broader scope or follow-up included in the Standard and Comprehensive programs.

This program is designed for families who need more than a focused snapshot but do not require the broader level of follow-up included in the Comprehensive program.
Expanded Support for Complex Aging-in-Place Needs
Our most comprehensive program is designed for older adults and families navigating multiple or evolving concerns related to mobility, home safety, everyday function, or a significant transition. It provides a broader level of in-home observation, practical planning, family education, and time-limited follow-up to help families make informed decisions and support safer aging in place.
This program is designed to help families move from uncertainty to a clearer, practical plan for supporting mobility, home safety, and independence.
All programs are private-pay, non-covered services. This model allows Dr. Alfonso to dedicate time to in-home observation, practical planning, family education, and individualized recommendations that fall outside the scope of traditional insurance-covered care.
These concierge programs are non-covered private-pay services and are not billed to Medicare or other insurance plans.
If a medically necessary physician service is separately provided, it may be billed to Medicare or other applicable insurance in accordance with coverage rules and regulations. Any separately billed physician service is distinct from the concierge program.
Program fees and billing arrangements are reviewed before enrollment. There are no hidden fees.
