A Physician-Led Perspective at Home
Mobility, everyday function, and home safety concerns can be difficult to understand fully during a brief office visit. Seeing the individual in the home can provide additional practical context about routines, supervision needs, mobility challenges, and the environment itself.
Dr. Mayra Alfonso is a physiatrist with over 30 years of clinical experience. She works alongside—not in place of—the individual’s primary care physician, specialists, therapists, and other existing professionals. Her role is to provide physician-led observation, identify practical priorities, and offer individualized written recommendations to support thoughtful planning for safety, independence, and aging in place.

What We Provide:
- Physician-led in-home observation: Dr. Alfonso reviews mobility, everyday function, supervision needs, and the home environment to help clarify practical concerns and priorities.
- Individualized written recommendations: Depending on the program selected, the family receives a clear written plan addressing home safety, mobility, daily routines, available support, and possible next steps.
- Time-limited follow-up: The Standard and Comprehensive programs include limited follow-up to answer questions, reinforce priorities, and support thoughtful planning.
The Home Safety & Viability Support Programs are private-pay, non-covered services. Any medically necessary physician service that is separately provided is distinct from the concierge program and may be billed to Medicare or other applicable insurance in accordance with coverage requirements.

Dr. Mayra Alfonso is a physiatrist with over 30 years of clinical experience across inpatient rehabilitation, outpatient practice, skilled nursing facilities, physician leadership, and physician-led home safety and aging-in-place support. She also brings more than eight years of experience working in skilled nursing facilities.
Her work focuses on helping older adults and families better understand concerns related to mobility, everyday function, home safety, and the ability to remain safely at home. She provides a thoughtful, practical perspective while working alongside the individual’s existing physicians, therapists, and other professionals.
