Her journey began with a deep curiosity about the mechanics of the human body and a passion for helping people recover from injury and illness. After completing her medical degree and residency, she embarked on a career that has spanned various settings, but her focus has always remained on restoring function and improving quality of life.

A Unique Perspective Forged in Experience

For over seven years, Dr. Alfonso served as a key physician leader within skilled nursing facilities. It was here that she witnessed firsthand the profound challenges that patients and their families face when navigating the healthcare system. She saw the consequences of fragmented care, the frustration of unanswered questions, and the deep desire of every patient to live as independently as possible.

This experience was formative. It became clear to Dr. Alfonso that there was a critical gap in the care continuum: a lack of expert, physician-led diagnosis and strategy during periods of change and transition, and a lack of unified guidance when families needed it most. She saw that the key to supporting older adults as they navigate aging in place was not just more care, but smarter, more coordinated care, led by a physician who understood the complete functional picture.

The Birth of a Concierge Practice

Driven by this insight, Dr. Alfonso founded her aging-in-place physiatry concierge practice. She stepped away from the high-volume, reactive model of traditional medicine to create a practice that allows her to do what she does best: spend time with patients, perform deep diagnostic assessments, and provide the thoughtful, personalized strategic guidance that families need.

Her practice is the culmination of a career spent on the front lines of geriatric rehabilitation. It is a model of care built on decades of experience, a passion for problem-solving, and an unwavering commitment to her patients’ independence.
