How Our Programs Support Families

Practical Clarity for Safer Aging in Place

Our programs are designed to help older adults and their families better understand concerns related to mobility, everyday function, home safety, and aging in place. Through in-home observation, practical recommendations, family education, and individualized planning, each program provides a different level of support based on the complexity of the situation and the family’s needs.

Across Our Programs, Families May Receive...

  • In-Home Observation: A focused review of mobility, everyday function, home safety concerns, and the practical challenges affecting independence at home.
  • Personalized Written Guidance: A physician-authored summary of key observations, priority concerns, safety considerations, and recommended next steps.
  • Family Education and Review: Dedicated time to discuss observations, answer questions, and help family members understand practical priorities.
  • Time-Limited Follow-Up Support: Available in selected programs to address questions and review meaningful changes as circumstances evolve.
  • Communication Support, When Appropriate: With authorization, relevant information may be shared with existing physicians, therapists, or other professionals.

Each program is structured differently so families can choose the level of support that best fits their circumstances.

Choose the Level of Support That Fits Your Needs

Each program offers a different level of support, from a focused one-time review to a more comprehensive plan with follow-up. The descriptions below can help you understand the differences, and the Complimentary Fit Screen will help determine which option may best match your circumstances.

All programs are private-pay. Program fees are reviewed during the Complimentary Fit Screen.

Priority Home Safety Snapshot

Focused Guidance for a Specific Concern

A focused, one-time program for older adults and families who have a specific concern related to home safety, mobility, or aging in place and need timely, practical direction. This program is designed to provide clarity around one primary issue without the broader scope or follow-up included in the Standard and Comprehensive programs.

What the Priority Home Safety Snapshot Includes

  • Focused In-Home Review: Observation of the specific mobility, home safety, or aging-in-place concern identified by the family.
  • Practical Recommendations: Clear, prioritized suggestions related to the primary concern.
  • Family Discussion: Time to review observations, answer questions, and discuss reasonable next steps.
  • Written Risk Snapshot: A concise physician-authored summary of the primary observations and recommendations.

This focused program is designed to provide practical clarity around one primary concern. It does not include ongoing follow-up or the broader level of support provided through the Standard and Comprehensive programs.

Standard Home Safety & Mobility Plan

A Broader View of Safety, Mobility, and Everyday Function

This program is designed for older adults and families who need a more detailed understanding of several concerns affecting safety, mobility, everyday function, or the ability to remain at home. It provides broader in-home observation, a personalized written plan, and family education to help clarify practical priorities and next steps.

What the Standard Home Safety & Mobility Plan Includes

  • Broader In-Home Review: Observation of mobility, everyday function, home safety concerns, and the practical challenges affecting independence at home.
  • Personalized Written Plan: A physician-authored summary of key observations, priority concerns, safety considerations, and recommended next steps.
  • Family Review and Education: Dedicated time to discuss observations, answer questions, and help family members understand practical priorities.
  • Focused Follow-Up: A limited follow-up opportunity to address questions and review meaningful changes after the initial visit.

This program is designed for families who need more than a focused snapshot but do not require the broader level of follow-up included in the Comprehensive program.

Understanding Costs & Payment

All programs are private-pay, non-covered services. This model allows Dr. Alfonso to dedicate time to in-home observation, practical planning, family education, and individualized recommendations that fall outside the scope of traditional insurance-covered care.

Regarding Medicare

These concierge programs are non-covered private-pay services and are not billed to Medicare or other insurance plans.

If a medically necessary physician service is separately provided, it may be billed to Medicare or other applicable insurance in accordance with coverage rules and regulations. Any separately billed physician service is distinct from the concierge program.

Program fees and billing arrangements are reviewed before enrollment. There are no hidden fees.
