A physiatrist is a physician who specializes in Physical Medicine and Rehabilitation (PM&R). Physiatrists focus on mobility, function, strength, balance, endurance, and how physical or medical conditions affect a person’s ability to carry out everyday activities. Dr. Alfonso brings this rehabilitation perspective to home safety and aging-in-place planning.
Dr. Alfonso does not replace your primary care physician. Your primary care physician remains responsible for your overall medical care, medications, preventive care, and acute health concerns. Dr. Alfonso provides a rehabilitation-focused perspective on mobility, everyday function, home safety, and aging in place. Her role is to help clarify practical concerns and provide individualized guidance that may fall outside the scope of a typical primary care visit.
Physical and occupational therapists provide skilled therapy focused on improving movement, function, and independence. Dr. Alfonso’s role is different: she brings a physician’s rehabilitation perspective to broader concerns involving mobility, everyday function, home safety, and aging in place. Her programs do not replace PT or OT, but may help families better understand priorities, identify practical concerns, and decide what next steps may be appropriate.
No. This is a non-emergency service. If you or your loved one are experiencing a medical emergency, including urgent symptoms after a fall or a sudden change in condition, please call 911 or go to the nearest emergency department immediately.
The in-home visit includes focused observation of mobility, everyday function, home safety, and the practical challenges affecting independence at home. Depending on the program and the individual’s needs, Dr. Alfonso may also review relevant history, observe movement and balance, discuss current concerns with the individual and family, and identify priorities for practical planning. The visit is designed to provide a clearer understanding of the situation in the person’s usual home environment.
Each program offers a different level of support. The Priority Home Safety Snapshot focuses on one specific concern and does not include ongoing follow-up. The Standard Home Safety & Mobility Plan provides a broader review, a personalized written plan, family education, and limited follow-up. The Comprehensive Home Safety & Mobility Plan is designed for more complex or evolving situations and includes the broadest level of review, planning, communication support, and time-limited follow-up. The Complimentary Fit Screen helps determine which option may best match the family’s circumstances.
The Home Safety & Viability Support Programs are private-pay, non-covered services and are not billed to Medicare or other insurance plans. Program fees and billing arrangements are reviewed before enrollment.
If a medically necessary physician service is separately provided, it may be billed to Medicare or other applicable insurance in accordance with coverage rules and regulations. Any separately billed physician service is distinct from the concierge program.
